Even after reasonable precautions are taken by the volunteers, officers, directors, coaches and instructors (collectively, the “Representatives”) of Active Together Family Sports, accidents, injuries and illnesses may occur to the participants of Active Together Family Sports for which Active Together Family Sports and its Representatives do not assume responsibility. By checking this box and in consideration of being permitted to participate in an Active Together Family Sports program, I hereby release, waive and forever discharge Active Together Family Sports, its Representatives and their heirs from any and all liability for any claims that may arise as a result of my participation or my child’s or children’s participation in an Active Together Family Sports program, including, but not limited to, claims related to medical, paramedical, hospital or dental services, save to the extent that such claims may be covered by any insurance coverage that Active Together Family Sports has in place. By checking this box, I also hereby consent to the use of my name or my child's name(s) and photographs in social media, newsletters, news reports and websites that promote Active Together Family Sports.
